Power of Attorney for Vehicle Transactions
Bill of Sale
Sworn Statement in Proof of Loss (Automobile).

BACKGROUND INFORMATION
Background: A 2021 Freightliner 114SD Dump Truck VIN# 3ALMG3DVXMDMJ0696 was damaged in a collision on February 14, 2023, and declared a total loss by the ACCA Liability Self-Insurance Fund, Inc. An offer for total loss in the amount of $171,703.70 has been offered by ACCA Liability Self-Insurance Fund, Inc. The Highway Department is requesting the Power of Attorney for Vehicle Transactions, Bill of Sale, and Sworn Statement in Proof of Loss (Automobile) be executed by the Chairman so ACCA will release the funds.
