Recommendation
Approve and authorize the Chairman to execute a revised Services Modification Proposal / Order Form with GovOS, Inc., in the estimated amount of $7,000.00 for the Probate Office Historical Marriage Record Upload Conversion.
The revised Services Modification Proposal / Order Form supersedes and makes null and void the previous proposal approved by the Commission on April 21, 2026.

BACKGROUND INFORMATION
Background: After the initial proposal was approved by the Commission on April 21, 2026, Probate staff received a revised Services Modification Proposal / Order Form from GovOS, Inc., with no change to the estimated amount of $7,000.00 for historical marriage record upload conversion services.
This project will be funded from Probate Data Processing Fund 721.
Previous Commission action/date: 04/21/2026 meeting: Approved and authorized the Chairman to execute the Proposal / Services Order Form with GovOS, Inc., in the estimated amount of $7,000.00 for the Probate Office Historical Marriage Record Upload Conversion.
