STAFF RECOMMENDATION
Recommendation
Take the following actions:
1) Terminate the Agency Agreement between the Baldwin County Commission and Symbol Health Solutions, LLC, for Employer Sponsored Near-site Clinics and Health Management; and
2) Notify Symbol Health Solutions, LLC, within the required ninety (90) days with written notice of termination as required in the contract.

BACKGROUND INFORMATION
Background: The Agreement states that either party may choose to terminate the agreement with written notice of such to the other party. Notice shall become effective upon mailing or receipt of such notice by the party to whom it is addressed.
Previous Commission action/date: 02/21/2017 meeting: The Baldwin County Commission, during its regularly scheduled meeting, accepted and approved the agreement to begin May 1, 2017.
